DD Waiver Services
We provide residential services to individuals diagnosed with developmental disabilities. At CSS , individuals are monitored 24/7. During this period , individual will obtain medication management daily, attend all schedule medical appointments and follow-ups, ensure their safety while in the community and make sure they are working towards their person center goals.
Mobile Crisis Response
We provide immediate support for individuals at risk of hospitalization due to their decline in mental health. We assist with coordinating medication management, medical evaluations and other community resources.
Crisis Community Stabilization
We provide support in stabilization mental health in the community by assisting with Activities of daily living, care coordination to medical, therapeutic and other providers and community resources for substance abuse, homelessness, abuse, employment and other services based on the individuals person centered plan.
